Like everybody else I too have been ripped off. I have been through all the same steps and phone calls to no avail. I currently have charges pending on my credit card despite filling in the supposed cancelation email form. All I can do now is inform my CCC on Monday and hopefully they will block all charges. The down side will be having to completely cancel my credit card, as other s have stated, they may just change their name once diet max is blocked. Be careful if you use this dentist office. I paid over $6, 000 for dental work. I succumbed to the very slick office practice of getting me to take out a credit card to cover the work. My fault!
